APPOINTMENT SCHEDULING: Self-Scheduling Appointment system is now available for you to access 24 hours a day. You can schedule and make changes to your appointment online by using the link below. When scheduling, please note if you need labs ordered before your appointment.

REFILL REQUESTS: If you have not been seen in the office in 6 months, please make a face-to-face appointment online to have your prescription refilled or updated. Otherwise, prescription requests should come from your pharmacy or by fax. Our new fax number is 910-338-0328. Please allow 3 business days to process your request.
